Obituary
Phyllis J. Cohen, 77, of Freeport, died Monday, January 7, 2013, at FHN Memorial Hospital.
Born May 18, 1935 in Freeport she was the daughter of Leonard L. and Elsie (Schmoldt) Dickman. She married Ralph Cohen May 17, 1953 in Kewanee, Illinois.
She was in the first class of Nurses with an Associate Degree from Highland Community College.
Phyllis was a registered nurse working with FHN Memorial Hospital, Rockford Memorial Hospital, and Swedish American Hospital.
Surviving are two sons, Jeffry A. (Diane) Cohen and Douglas N. (Lisa Johnson) Cohen both of Rockford; one grandchild, Sara Cohen of Rockford.
She was preceded in death by her parents and one brother.
Funeral services will be 1 p.m. Monday, January 14, 2013 in the Chapel of Peace at Chapel Hill Memorial Gardens.
There will be no visitation
In lieu of flowers a memorial has been established for Friends Forever Humane Society.
